수면 단계의 판독-7(sleep stage R-1)

 

Stage R

1. Score in accordance with the following definitions;

Rapid eye movements (REMs): Eye movements recorded in the EOG derivations consisting of conjugate, irregular, sharply peak eye movements with an initial deflection usually lasting <500 msec. While rapid eye movements are characteristic of stage R sleep, they may also be seen in wakefulness with eyes open when individuals visually scan environment.

Low chin EMG tone: Baseline EMG activity in the chin derivation no higher than in any other sleep stage and usually at the lowest level of the entire recording.

Sawtooth waves: An EEG pattern consisting of trains of sharply contoured or triangular, often serrated, 2-6 Hz waves maximal in amplitude over the central head regions and often, but not always, preceding a burst of rapid eye movements.

Transient muscle activity: Short irregular bursts of EMG activity usually with duration <0.25 seconds superimposed on low EMG tone. The activity may be seen in the chin or anterior tibialis EMG derivations, as well as in EEG or EOG deviations, the latter indicating activity of cranial nerve innervated muscle (facial and scalp muscle). The activity is often maximal associated with rapid eye movements.

 

1. 다음의 정의에 따라 Stage R로 판독할 수 있다.

 

급속안구운동 (REMs): EOG에서 기록된 눈 움직임은 일반적으로 500 msec 미만으로 지속되는 빠른 초기편향을 가진 불규칙하고 날카로운 피크의 눈 움직임을 보인다. 급속 안구운동은 stage R의 특징이지만 Stage W와 같이 시각적으로 주변환경을 보거나 눈을 뜬 상태에서도 나타날 수 있다.

 

Low chin EMG tone: 다른 수면 단계와 비교해서 전체적인 수면 단계보다 가장 낮은 수준의 EMG activity를 보인다.

 

Sawtooth waves: 톱니모양으로 나타나는 파형은 central에서 진폭이 최대인 날카로운 삼각형 모양으로 2-6 Hz로 구성된 EEG이며 종종 빠른 눈 움직임이 선행하지만 항상 그런 것은 아니다.

 

Transient muscle activity: 일시적인 근육활동으로 일반적으로 지속시간이 0.25초 미만인 짧고 불규칙한 EMG activit로 턱이나 앞정강근뿐 아니라 뇌신경과 연결된 얼굴이나 두피근육, EEG, EOG에서도 나타날 수 있다. 이 활동은 때로 빠른 눈 움직임과 관련해 최대로 나타날 수 있다.

 

2. Score stage R sleep in epochs with all of the following phenomena (definite stage R):

a.Low amplitude, mixed frequency EEG activity without K complexes or sleep spindles

b.Low chin EMG tone for the majority of the epoch and concurrent with REMs

c.REMs at any position within the epoch

 

3. Score segments of sleep preceding and contiguous with an epoch of definite stage R (as defined in rule I.2), in the absence of rapid eye movements, as stage R if all of the following are present: (see Figure 11, 12 and 13)

a.The EEG shows low amplitude mixed frequency activity without K complexes or sleep spindles.

b.The chin EMG tone is low (at the stage R level).

c.There is no intervening arousal. (see Figure 12C)

d.Slow eye movements following an arousal or stage W are absent.

 

2. 다음 현상이 있는 epoch를 stage R로 scoring 할 수 있다.

a. 수면 2단계의 특징인 K complex와 sleep spindle 이 없는 저진폭 혼합주파수(LAMF)의 EEG activity.

b. 대부분의 epoch에서 낮은 수준의 턱 EMG와 급속안구운동이 동반됨.

c. 하나의 epoch내에 급속안구운동이 나타날 때

 

3. REM이 보이지 않는 다음 epoch에서는 만일 EEG에서  LAMF가 지속되고 K complex와 sleep spindle이 보이지 않고 chin EME가 지속적으로 최저인 상태여야 하며, arousal이나 stage W의 소견이 없다면 없다면 지속적으로 stage R로 판독한다.

 

Stage R의 시작

A. 52 epoch는 stage R. 53 epoch는 REM의 지속 규칙에 따라 Stage R로 판독한다

B. 61 epoch는 이전 epoch에서 나타나던 알파파가 사라지고 LAMF EEG activity가 나타나고 SEM이 epoch의 후반부에 나타나므로 stage N1. 62 epoch는 LAMF가 지속되고 그다음 63 epoch에서 REM이 관찰되므로 62와 63은 stage R로 판독한다.

C. 71 epoch 은 LAMF, 72 epoch 역시 LAMF, 73 epoch의 전반부에서 REM이 관찰되므로 71, 72, 73 모두 stage R로 판독한다.
