REM 수면의 시작은 어떻게 정의하는가?

A.Transition from definite stage N2 (epoch 50) to definite stage R (epoch 53). The EEG of the majority of epoch 51 and all of epoch 52 has low amplitude, mixed frequency (LAMF) EEG without sleep spindles or K complexes and EMG is at the stage R level. As the epoch 51 and 52 are contiguous with definite stage R (Epoch 53), they are second as stage R.

B.Epoch 60 is an epoch of definite stage N2. Epoch 61 is scored as stage N2 by the stage N2 continuation rule. Epoch 62 and 63 are scored as stage R as the EEG has LAMF activity without K complexes or sleep spindles, the chin EMG is at the stage R level, and the epochs are contiguous with an epoch of definite stage R (epoch 64). Note that, using G.2, epoch  62 would be scored as stage N2. However, the stage R rule (I.3) takes precedence.

C.Using rule G.6.b, epoch 71 would be scored as stage N1 (arousal ends stage N2). However, REM rule I.3 takes precedence and epochs 72 and 73 are scored as stage R.

 

 

A. 전환

확실한 2단계 수면인 50번째 epoch에서 확실한 stage R로 53 epoch에서 전환이 이루어 진다. EEG와 EMG 는 51 epoch의 대부분과 52 epoch는 k complex나 sleep spindle 이 없는 LAMF의 EEG가 보이며, EMG는 stage R의 수준으로 매우 낮은 상태이다. 따라서 51번째와 52번째 epoch는 확실한 stage R을 시사하는 52 epoch와 인접해 있으므로 stage R로 스코어링 한다. 51 epoch의 초반부에는 stage N2를 시사하는 K complex가 보이지만 그 이후로 LAMF와 Low chin EMG activity가 보이므로 stage R의 시작을 51 epoch 로 할 수 있다.

B. stage N2와 stage R의 구분 

60 epoch는 확실한 N2단계이다.  61 epoch는 stage N2의 지속 규칙에 따라 N2로 scoring 한다. 62, 63 epoch는 EEG 상에서 K complex나 sleep spindle이 없는 LAMF가 나타나며 EMG는 stage R단계에 합당하다. 확실한 stage R 을 보이는 64 epoch와 인접해 있으므로 stage R로 scoring 한다. 62 epoch는 stage N2라고 할 수도 있지만 stage R단계 규칙이 우선하므로 stage R로 판독한다.

C. arousal 이후의 scoring

70 epoch에서 stage N2 sleep이 이어져 오다가 71 epoch의 초반에 arousal이 발생했으므로 N2단계의 수면은 종료되고 stage N1으로 scoring 할 수 있다. 그러나 stage R의 규칙이 우선하므로 72, 73 epoch는 stage R로 판독한다.

B.A transition between definite stage N2 (epoch 60) and definite stage R (epoch 63). Stage N2 is considered to continue until the last K complex or sleep spindle.

C.Epoch 72 is scored as stage R as the majority of epoch 72 (following the sleep spindle in the first half of the epoch) has an EEG with LAMF activity without K complexes or sleep spindles, the chin EMG is at the stage R, and this portion of the record is contiguous with definite stage R (epoch 73). Note that, by rule G.2, epoch 72 would be scored as stage N2. However, rule I.3 takes precedence over rule G.2. As the majority of epoch 72 meets rule I.3 criteria, the epoch is scored as stage R.

A. 확실한 N2 sleep 인 50 epoch에서 확실한 stage R 인 53 epoch로의 전환이 이루어 진다. 52 epoch는 k complex나 sleep spindle이 없는 LAMF를 보이고 51 epoch의 끝에서 EMG가 stage R 수준으로 떨어졌으므로 stage R로 판독한다.

B. 확실한 N2 sleep 인 60 epoch에서 확실한 stage R인 63 epoch로의 전환이 이루어 진다. N2 단계는 마지막 K complex나 sleep spindle이 나타날 때 까지 지속되는 것으로 간주한다.

C. 72 epoch는 K complex나 sleep spindle이 없는 LAMF가 우세하게 나타나고 EMG가 stage R수준으로 떨어져 있으며 그 다음 73 epoch에서 REM과 EMG가 최저로 낮아져 있으므로 확실한 stage R을 보이므로 stage N2가 끝나는 72 epoch의 sleep spinedle이후 부터 stage R로 판독한다.

 

4. If the majority of an epoch contains a segment of the recording meeting criteria for stage R (rule I.2, I.3, I.5), the epoch is scored as stage R. Stage R rules takes precedence over stage N2 rules. (see figure 12, epoch 62 and figure 13, epoch 72)

5. Continue to score segments of  sleep that follow one or more epochs of definite stage R (as defined in I.2), in the absence of rapid eye movements, as stage R if all of the following are present: (see Figure 14-18)

a.The EEG show is low amplitude, mixed frequency activity without K complexes or sleep spindles

b.The chin EMG tone is low (at the stage R level) for the majority of the epoch

There is no intervening arousal

 

4. epoch의 대부분이 stage R기준을 충족하는 경우, 해당 epoch는 stage R 단계로 scoring 한다. stage R의 규칙은 stage N2 보다 우선한다.(Figure 12의 62 epoch, Figure 13의 73 epoch)

5. 확실한 stage R이후 급속안구운동이 없는 경우 다음 조건이 모두 충족되면 stage R로 판독한다

a.EEG가 K complex나 sleep spindle이 없는 LAMF activity가 보여야 한다

b.epoch의 대부분에서  EMG tone이 stage R 수준으로 낮아야 한다

c.중간에 arousal이 없어야 한다.
