수면 단계의 판독-9(sleep stage R-3, REM 수면의 지속과 끝에 관한 내용)

REM 수면은 어떻게 지속되는 걸 판단하고, 언제 끝나는가?

A.Epoch 50 is definite stage R (rule I.2). Epochs 51 and 52 continue to be scored as stage R as the chin EMG remains low and the EEG does not contain K complexes or sleep spindles. Epoch 53 is scored as stage R as the chin EMG tone does not increase until the last half of the epoch. Epoch 53 is scored as stage N2 given that the K complex is unassociated with an arousal in the first half of the epoch.

B.Epoch 60 is definite stage R and Epoch 61 continues to be scored as stage R as the chin tone remains low and the EEG contains low amplitude, mixed frequency activity. Epoch 62 is not scored as stage R as the chin tone is not low for the majority of the epoch. Epoch 62 is scored as stage N1 given the low amplitude mixed frequency EEG pattern and the absence of K complexes or sleep spindles in the first half of the epoch.

 

A. 50 epoch는 LAMF와 REM, stage R수준의 chin EMG로 stage R. 51, 52 epoch는 chin EMG가 낮고 EEG상 K complex나 sleep spindle이 없기 때문에 stage R로 지속해서 scoring 한다. 53 epoch는 chin EMG tone이 epoch의 후반부까지 증가하지 않기 때문에 stage R로 판독하지만 K complex가 arousal과 관련이 없기 때문에 stage N2로 판독한다.

B. 60 epoch는 확실한 stage R. 61 epoch는 chin EMG가 낮고 EEG가 LAMF를 보이고 있어 지속적으로 stage R로 판독한다. 62 epoch는 chin EMG tone이 epoch의 50% 이상 높게 나타나기 때문에 stage R로 판독할 수 없다. LAMF의 EEG와 K complex나 sleep spindle이 없는 것을 고려하여 stage N1으로 판독하고, 63 epoch의 초반에 K complex가 나타나므로 stage N2로 판독한다.

A.Stage R is interrupted by an arousal followed by low-amplitude, mixed frequency EEG without slow eye movements. Epoch 52 continues to be scored as stage R as the EEG shows a low-amplitude, mixed frequency pattern, and the majority of the epoch contains low chin EMG tone (at the stage R level). Compare the effects of an arousal interrupting stage R with one interrupting N2 (Figure 8).

B.Stage R is interrupted by an arousal followed by slow eye movements and low amplitude, mixed frequency. Because slow eye movements are present following the arousal, Epoch 62 is scored as stage N1 even though chin activity is at the stage R level. Stage N1 continues until there is evidence for another sleep stage (here stage N2 in epoch 63).

C.Stage R is interrupted by an arousal followed by slow eye movements and low amplitude mixed frequency EEG. However, the majority of epoch 72 does not contain slow eye movements and is scored as stage R based on rule I.3 as epoch 73 is definite stage R.

D.Stage R in interrupted by an arousal followed by slow eye movements and low amplitude mixed frequency EEG with chin activity at the stage R level. Epoch 82 is scored as stage N1 even if the chin activity is at the stage R level as the majority of the epoch contains slow eye movements. Stage N1 continues until there is evidence of another sleep stage (here definite stage R in epoch 83).

 

A. Stage R은 arousal에 의해 방해받지만 LAMF가 나타나고 chin EMG가 여전히 최저로 낮기 때문에 52 epoch까지 stage R로 판독한다. 그 후 53 epoch의 초반에 Kcomplex가 나타나고 EMG tone이 증가했으므로 stage N2로 판독한다.

B. Stage R은 느린 안구운동(SEM)과 LAMF에 의해 방해받는다. SEM이 존재하기 때문에 chin EMG가 낮음에도 불구하고 62 epoch는 N1이며, 63 epoch는 k complex와 chin EMG tone의 증가로 stage N2로 판독.

C. Stage R이 각성 후 느린 안구운동과 LAMF에 의해 방해받지만 epoch의 대부분에서 느린 안구운동이 없고 72 epoch의 초반에 각성과 함께 SEM이 보이나 지속적으로  chin EMG tone이 감소하고 73 epoch에서는 다시 REM이 관찰된다.

D. Stage R이 각성 후 느린 안구운동과 LAMF에 의해 방해받고 chin EMG tone이 낮은 수준을 유지하지만 82 epoch의 대부분에서 SEM이 관찰되므로 stage N1으로 판독하고 83 epoch의 초반에 다시 REM이 나타나므로 stage R로 판독한다.

 

Stage R의 유지와 종료

A.Epoch 52 continues as stage R as the EEG contains low amplitude mixed frequency activity, the chin EMG tone is low, and slow eye movements do not follow the major body movement. Note that if Epoch 51 was scored as stage W based on the appearance of alpha activity, stage R would end (movement rule in section J).

B.Epoch 62 is not scored as stage R even if the EEG exhibits low amplitude mixed frequency activity and the chin tone remains low because slow eye movement.

 

A. LAMF의 EEG activity가 나타나고 chin EMG tone이 낮으며 몸의 움직임 이후에 SEM이 나타나지 않기 때문에 52 epoch는 stage R로 계속 판독한다.

B. LAMF의 EEG activity가 나타나고 chin EMG tone이 낮더라도 몸의 움직임 이후에 SEM이 나타나기 때문에 61 epoch부터 stage N1으로 판독한다.

 

A. stage R에서 N2로 변환됨. 52 epoch의 전반부에 K complex로 N2를 시사함.

B. stage R이 지속되다가 62 epoch의 후반부에 K complex로 stage R로 판독하고 그다음 epoch부터 N2로 판독한다.

 

REM과 K complex가 혼재해 있을 때

 

A. 50 epoch에서 R이 차지하는 비중이 크므로 R로 판독하고 53에서 REM이 나왔으므로 stage R의 지속규칙에 따라 R로 판독한다.

B. 61 epoch의 초반 k complex 이후 R 61의 후반에  k complex가 보이므로 각 epoch에서 차지하는 portion이 stage R이 우세함. 

C. 70 epoch의 R이 차지하는 비중이 크므로 R 71의 전반부에 chin EMG 증가 후반부에 k complex로 71은 N1이 더 많이 차지하므로  N1이고, K complex 이후로 N2로 이어지며, 73에서 Low chin EMG와 REM.

D. 80 epoch R이 우세, 80의 후반 k com 81의 후반 k com으로 81은 N2, 83에서 REM과 Low chin EMG이고 81의 K complex전까지 R로 판독한다.

 

Stage R이 종료되는 조건들

 

6. End of scoring stage R sleep with one or more of the following occur;

a.There is a transition to stage W or N3.

b.An increase in chin EMG tone above the level of stage R is seen for the majority of the epoch and criteria for stage N1 are met.

c.If an arousal interrupts stage R, score the portion of the record containing slow eye movements with low amplitude mixed frequency EEG and no posterior dominant rhythm as stage N1 even if the chin EMG activity remains low (at stage R level). Continue to score stage N1 as long as there are slow eye movements until there is evidence for another stage, usually stage N2 or stage R. If there are no slow eye movements and chin EMG tone remains low, continue to score as stage R.

d.There is a major body movement followed by slow eye movements and low amplitude mixed frequency EEG without non arousal associated K complexes or sleep spindles (score the epoch following the major body movement as stage N1; if no slow eye movements are present and the EMG tone remains low, continue to score as stage R; the epoch containing the body movement is scored using the criteria in section J.

e.One or more non arousal associated K complexes or sleep spindles are present in the first half of the epoch in the absence of rapid eye movements, even if chin EMG tone remains low (score the epoch stage N2).

 

a. Stage R에서 stage W 또는 stage N3으로 전환되면 stage R은 종료된다.

b. epoch의 대부분에서 chin EMG tone이 stage R수준을 초과하고 stage N1단계의 기준으로 변하면 stage R은 종료된다.

c. Stage R이 지속되다가 aruosal이 발생해 방해를 받으면 SEM과 LAMF가 있는 부분은 chin EMG tone이 낮더라도 stage N1으로 판독한다. SEM이 지속되면 N1으로 계속 판독하고 N2나 R과 같이 다른 단계의 수면을 나타내는 증거들이 나타날 때까지 N1으로 판독한다. SEM이 없고 chin EMG tone이 낮으면 계속 stage R로 판독한다.

d. 몸의 움직임 후 SEM과 LAMF가 나타나고 K complex나 sleep spindle이 없으면 body movement 이후 epoch는 stage N1으로 판독한다. SEM이 없고 chin EMG tone이 낮으면 stage R로 계속 scoring 한다.

e. 비각성과 관련된 K complex나 sleep spindle, epoch의 첫 번째 절반에 비각성과 관련된 K complex나 sleep spindle이 나타나고 빠른 안구운동(REM)이 없으면 chin EMG tone이 낮더라도 해당 epoch는 stage N2로 판독한다.

 

7. Score segments of the record with low chin EMG activity and a mixture of REMs and sleep spindles and/or K complexes as follows;

a.Segments between two K complexes, two sleep spindles, or a K complex and sleep spindle without intervening REMs are considered to be stage N2.

b.Segments of the record containing REMs without K complexes or sleep spindles and chin tone at the REM level are considered to be stage R.

c.If the major of an epoch contains a segment considered to be stage N2, it is scored as stage N2. If the majority of an epoch contains a segment considered to be stage R, it is scored as stage R.

 

 낮은 수준의 chin EMG와 REM 및 sleep spindle 또는 K complex의 혼합이 있는 부분에서의 scoring 

a. 두 개의 K complex나 두 개의 sleep spindle, 또는 K complex와 spindle 사이에 REM이 없는 경우 stage N2로 판독한다.

b. K complex나 sleep spindle이 없는 REM이 포함되고 chin EMG tone이 stage R에서 정도로 낮은 경우 stage R로 판독한다.

c. epoch의 대부분이 N2로 간주되는 분절을 포함하면 N2로 판독한다. epoch의 대부분이 stage R로 간주되는 분절을 포함한다면 stage R로 판독한다.

 

J. Scoring Epochs with Major Body Movements

1.Score in accordance with the following definition:

Major body movement: Movement and muscle artifact obscuring the EEG for more than half an epoch to the extent that the sleep stage cannot be determined.

2. If posterior dominant rhythm is present for part of epoch (even <15 seconds duration), score as stage W.

3. If no posterior dominant rhythm is discernible, but an epoch scoreable as stage W either precedes or follows the epoch with a major body movement, score as stage W.

4. Otherwise, score the epoch as the same stage as the epoch that follow it.

 

움직임이 있는 부분에서의 scoring

1. major body movement는 EEG를 1개의 epoch의 절반이상 가릴 정도로 움직임과 근육의 아티팩트가 발생하여 수면단계를 결정할 수 없는 경우를 의미한다.

2. epoch의 일부(15초 미만일지라도)에서 PDR 이 나타나면 해당 epoch는 stage W로 판독한다.

3. PDR이 없는 경우, PDR이 식별되지 않지만 W로 판독할 수 있는 epoch가 body movement가 있었던 epoch의 이전이나 이후에 존재한다면 해당 epoch는 W로 판독한다.

4. 위의 조건이 모두 해당하지 않는 경우, 해당 epoch는 그다음 epoch와 동일한다 단계로 판독한다.
