수면 단계의 판독-6(sleep stage N2, N3)

 

Stage N2

 

4. Continue to score epochs with low-amplitude, mixed frequency EEG activity without K complexes or sleep spindle as stage N2 if they are preceded by epochs containing either of the following and there is no intervening arousal:

a.K complex associated with arousal

b.Sleep spindle

5. Epohs following an epoch of the stage N3 that do not meet criteria for stage N3 are scored as stage N2 if there is no intervening arousal and the epoch dose not meet criteria for stage w or stage R.

 

4. K complex나 sleep spindle 이 없는 낮은 진폭의 혼합주파수 EEG activity가 보이는 epoch는 그 이전에 K complex가 arousal과 관련되어 있거나 sleep spindle이 있는 epoch가 존재하고, 그 사이에 각성이 없다면 N2로 판독한다.

5. N3단계의 epoch다음 N3기준을 충족하지 못하는 epoch는 만일 그 사이에 arousal이 없고 해당 epoch가 stage W, stage R을 만족하지 않는다면 N2로 판독한다.

 

6. End scoring stage N2 sleep when ONE of the following events occurs.

a.Transition to stage W

b. An arousal followed by low-amplitude, mixed frequency EEG(change to stage N1 until a K complex unassociated with an arousal or sleep spindle occurs). This assumes that the epoch does not meet criteria for stage R(see rule I.3 and Figure 12C).

c.A major body movement followed by slow eye movements and low amplitude, mixed frequency EEG without non-arousal associated K complex or sleep spindles (score the epoch following the major body movement as stage N1; score the epoch as stage N2 if the are no slow eye movements; the epoch containing body movement is scored using the major body movement rules under section J).(see Figure 10)

d. Transition to stage N3

e. Transition to stage R

 

6. StageN2의 종료는 다음과 같은 조건 중 하나가 있을 때 이다.

a. Stage W로 바뀔때.

b. LAMF의 EEG activity를 동반한 arousal이 있을 때(arousal이 없거나 k complex 또는 sleep spindle이 없는 경우 N1으로 판독), 이 경우  해당 epoch이 stage R을 충족하지 않는다는 가정하에 stage N2로 판독한다.

c. 몸의 움직임 후 느린 안구운동(SEM)과 LAMF의 EEG activity가 나타나고 K complex나 sleep spindle이 없는 경우(major body movement이 후는 stage N1으로 판독하고 SEM이 없으면 stage N2로 판독한다.

d. Stage N3로 바뀔때

e. Stage R로 바뀔때

A.Epoch 52 continues to be scored as stage N2 as the major body movement is NOT followed by slow eye movements. Epoch 51 is scored according to the major body movement rules (section J). As epoch 51 does not contain alpha activity and an epoch of stage W does not precede or follow the epoch, the major body movement is scored the same as the epoch that follows it (stage N2; rule J.4).

B.Epoch 62 is scored as stage N1 (stage N2 ends following the major body movement) as the body movement is followed by slow eye movements and low-amplitude, mixed frequency EEG(rule G.6.c). Epoch 63 is scored as stage N2 as a K complex unassociated with an arousal occurs in the first half of the epoch.

A.52 epoch는 body movement 이후에 SEM이 나타나지 않기 때문에 Stage N2로 계속 scoring 한다. 51 epoch는 movement rule에 따라 판독하고, epoch 51에는 alpha리듬이 없고 stage W의 epoch이 51의 앞뒤로 없기 때문에 body movement는 그 뒤에 epoch와 동일하게 N2로 scoring 한다.

 

B.62 epoch는 body movement에 뒤이어 SEM과 LAMF를 시사하는 EEG activity 가 나타나므로 stage N1으로 판독한다. 63 epoch는 arousa과 관련이 없는 K complex가 epoch의 전반부에 나타나므로 다시 N2로 판독한다.

 

Stage N3

1.Score in accordance with the following definition;

Slow wave activity: Waves of frequency 0.5 Hz-2 Hz and peak to peak amplitude >75 ㎶, measured over the frontal regions referenced to the contralateral ear or mastoid (F4-M1, F3-M2)

2. Score stage N3 when ≥20% of an epoch consists of slow wave activity, irrespective of age.

 

Stage N3는 다음의 정의에 따라 판독한다.

1. Slow wave activity: 주파수가 0.5~2Hz 사이이며, 파형의 최고 정점에서 저점까지의 진폭이 75 ㎶ 이상이어야 한다. 이 파형은 F4-M1, F3-M2의 뇌파유도에서 측정한다.

2. stage N3는 epoch의 20%인 6초 이상 slow wave activity가 나타날 경우 N3로 판독하며 기준은 나이에 관계없이 적용된다.

 

Note 1. Stage N3 represents slow wave sleep and replaces the Rechtschaffen and Kales nomenclature of stage 3 and stage 4 sleep.

Note 2. K complexes would be considered slow waves if they meet the definition of slow wave activity.

Note 3. Pathological wave forms that meet the slow wave activity criteria, such as those resulting from neurological impairment, encephalopathy, or epilepsy, are not counted as slow wave activity of sleep. Similarly, waveforms produced by artifact or those of non-cerebral origin should not be included in the scoring of slow waves.

Note 4. Sleep spindles may persist in stage N3 sleep.

Note 5. Eye movements are not typically seen during stage N3 sleep.

Note 6. In stage N3, the chin EMG is of variable amplitude, often lower than in stage N2 sleep, and sometimes as low as in stage R sleep.

Note 7. Due to reported difficulty correctly identifying qualifying slow waves, >75 ㎶ lines inserted in the recording or identification of delta waves by concurrent digital analysis, may assist the scorer. Candidate slow waves may by marked such that the total duration of slow waves may be quantified.

 

1.Stage N3는 느린 뇌파 활동을 나타내며 stage 3과 4는 stage N3으로 명명한다.

2.K complex는 slow wave activity를 보이는 경우 slow wave로 고려할 수 있다.

3.신경학적 손상이나 뇌병증 또는 뇌전증으로 인한 병리적인 파형은 slow wave activity를 충족하더라도 N3로 판독하지 않는다. 또한 artifact나 뇌가 아닌 다른 원인으로 생성된 파형도 느린 파형에 속하지 않는다.

4.Sleep spindle은 staege N3에서도 나타날 수 있다.

5.Stage N3에서는 안구운동이 관찰되지 않는다.

6.Chin EMG는 가변적인 진폭을 가지며, 종종 stage N2 보다 낮고 때로는 stage R과 비슷하게 나타날 수도있다.

 

7.75 ㎶ 이상의 Slow wave는 정량활 할 수 있도록 표시할 수 있다.
