수면 단계의 판독-5(sleep stage N2)

Stage N2

 

1.Score in accordance with the following definitions

K complex: A well delineated, negative, sharp wave immediately followed by a positive component standing out from the background EEG, with total duration ≥0.5 seconds, and usually maximal in amplitude when recorded using frontal derivations. For an arousal to be associated with a K complex, the arousal must either be concurrent with the K complex or commence no more than 1 second after termination of the K complex

Sleep spindle: A train of distinct sinusoidal waves with frequency 11-16 Hz (most commonly 12-14 Hz), with a duration ≥0.5 seconds, and usually maximal in amplitude in the central derivations.

 

1. 다음의 정의에 따라 scoring 한다.

K complex: K complex는 background EEG에서 두드러지게 나타나는 negative sharp 모양과 positive 성분이 뒤따라 나타나며 지속시간(duration)은 0.5초 이상이어야 하고, 일반적으로 뇌파의 frontal 유도에서 최대 진폭을 보인다. K complex와 관련된 arousal 은 K complex와 동시에 발생하거나 K complex가 나타나고 1초 이내에 나타나야 한다.

 

Sleep spindle: sleep spindle은 주파수가 11-16Hz (일반적으로 12-14Hz)의 뚜렷한 사인파형이 연속적으로 나타나며, 지속시간이 0.5초 이상이어야 하고, 일반적으로 뇌파의 Central 유도에서 최대진폭을 보이는 파형이다. 

 

2. Begin scoring stage N2 (in absence of criteria for stage N3) if either or both of the following occur during the first half of the epoch or the last half of the previous epoch.

a.One or more K complexes unassociated with arousals.

b.One or more sleep spindles

 

3. Score a given epoch as stage N2 if the majority of the epoch meets criteria for stage N2. If the waveforms in rule G.2.a or G.2.b are followed by an arousal in the same or subsequent epoch (see rule G.6.b)

 

2. N3 수면단계가 보이지 않는 경우 첫 번째 epoch의 절반 또는 이전 epoch의 마지막 절반의  EEG 동안 다음 중 하나 이상이 발생하면 stage N2로 판독한다.

a. 각성과 관련이 없는 하나 이상의 K complex

b. 하나 이상의 sleep spindle

 

3. epoch의 대부분이 stage N2를 충족하면 해당 epoch는 stage N2로 판독한다. 또한 2번의 조건에서 언급했듯 각성 없이 나타나는 하나 이상의 K complex 또는 sleep spindle 이 보일 때에도 stage N2로 판독한다.

 

Start and continuation of stage N2. The EEG is assumed to show LAMF unless otherwise depicted.

A.Start of N2. Epoch 50 is scored as stage N2 as there is a K complex (unassociated with an arousal) in the first half of the epoch(rule G.2) Epoch 51 is stage N2 as this stage continues for the majority of the epoch(rule G.3). Following an arousal, epoch 52 is scored as stage N1 (rule G.6.b) until there is evidence for another stage of sleep. A k complex is noted in the last half of epoch 52 and epoch 53 is scored as stage N2 (rule G.2)

B.At the end epoch 61, there is a k complex associated with an arousal, Epoch 62 is scored as stage N1 (rule G.6.b). A k complex associated with an arousal is not considered evidence for stag N2. Epoch 63 is scored as stage N2 by rule G.2

C.A k complex occurs in the last half of epoch 70 but stage N2 is considered to be present only up to the arousal in epoch 71. Epoch 71 is scored as stage N1 as the majority of the epoch follows the arousal. Epoch 72 scored as stage N1 as the k complex does not occur until the second half of the epoch.

 

Stage N2의 시작과 지속에 대한 설명으로 각 epoch에 대해 설명한다.

A. 50 epoch은 K complex가 나타나고 arousal과 관련이 없으므로 stage N2로 판독한다. 51 epoch은 앞서 나타난 stage N2가 이어지며 epoch의 중반이상을 차지하므로 stage N2로 판독한다. 52 epoch는 51 epoch의 후반부에 발생한 arousal로 인해 수면 단계가 stage N1으로 판독하고, 52 epoch의 후반부에 다시 k complex 가 나타나지만 arousal이후 K complex 가 나타나기 전까지의 N1이 지속된 시간이 더 많은 portion을 차지하므로 stage N2로 판독하지 않는다.  

B. 60 epoch의 초기에 k complex가 나타나서 2단계 수면이 유지되고, 61 epoch에서 K complex와 함께 arousal이 발생하여 N1으로 판독하고 각성과 관련된 K complex는 N2로 판독하지 않는다.

62  epoch의 중 반에 K complex가 시작하여 63 epoch까지 같은 stage N2의 수면에 지속된다.

C. 70 epoch의 초반에 K complex로 N2, 후반부에 K complex가 나타나지만, 71 epoch에서 aruosal 발생하여 N1으로 판독하고 71 epoch aruosal 이후 N2를 시사하는 단계의 뇌파가 보이지 않으므로 N1으로 계속판독한다. 72 epoch의 후반에 K complex가 나타나므로 N1이 차지하는 비중이 더 크므로 N1으로 판독한다. 72의 후반에 K comple가 나왔으므로 지속적으로 73까지 N2로 판독한다.
